Bridges Project Connect

Bridges Project Connect, part of Bridges Outreach, helps people experiencing homelessness move toward permanent housing and greater stability. Its work combines street outreach, intensive case management, housing navigation and placement, and housing sustainment support.

Outreach staff connect with people living outdoors or in other places not meant for habitation, providing food and essential items while helping connect them with shelter and other services. Through Project Connect, case managers assess individual needs and help develop housing stabilization plans. Support may include obtaining identification documents, enrolling in benefits, connecting with healthcare, pursuing housing vouchers, and addressing barriers to housing.
