Homeward Bound’s Permanent Supportive Housing Program is dedicated to providing safe, stable housing paired with comprehensive, long-term case management services for highly vulnerable, chronically homeless individuals. The program specifically serves people who have experienced homelessness for at least a year and are living with one or more disabling conditions. These conditions often include mental illness, substance use disorders, and physical disabilities, making it difficult for them to maintain stable housing without additional support.
Recognizing the complex needs of this population, Homeward Bound offers intensive care and personalized assistance to help residents not only secure housing but also maintain their independence and improve their overall quality of life. By addressing both housing and health-related challenges, the program aims to break the cycle of homelessness and foster long-term stability and well-being.
