Description
H4 Medical Services operates a satellite walk-in clinic in Kaneohe to serve the Windward side of Oahu, stretching from Hauula to Waimanalo. Located conveniently in the Kaneohe Civic Center alongside state assistance offices, the clinic is staffed by a physician or advanced practice registered nurse (APRN), supported by a medical assistant and care coordinator.
The clinic provides comprehensive primary care services, including medication refills, treatment for allergic reactions, asthma attacks, burns, bronchitis, cold and flu, fevers, minor procedures like stitches, lacerations, muscle strains, viral illnesses, TB tests, vaccinations (MMR, tetanus), and care for other common illnesses. Additionally, H4 offers treatment for mental illness and substance abuse either directly or via referrals.
Recognizing the geographic spread and cultural barriers faced by homeless individuals along the Windward coast, H4 operates a street medicine program. An APRN trained in street medicine travels to known homeless encampments with an outreach specialist and medical assistant, providing on-site care, prescribing medications, and updating electronic medical records remotely.
In spring 2021, H4 launched an interim medical clinic at the Punawai Building (431 Kuwili Street) on the edge of Chinatown. This clinic offers primary care services from 10 a.m. to 6 p.m., seven days a week, staffed by a physician or APRN, an RN, and at least one medical assistant. While building out the permanent clinic space, H4 provides street medicine services using a van parked outside the Punawai Rest Stop, supplemented by telemedicine.
The permanent Punawai Clinic is expected to open in summer 2021 with expanded hours and enhanced facilities, including six fully enclosed exam rooms, spaces for ancillary services, patient counseling, waiting areas, and administrative offices. This expansion will allow H4 to better serve Oahu’s homeless and underserved populations with accessible, high-quality medical care.
