Description

We are honored to be designated by the U.S. Department of Housing and Urban Development (HUD) to lead a comprehensive response to homelessness in our community. Our organization coordinates crucial funding and oversees the delivery of housing and supportive services designed specifically for people experiencing homelessness. As part of our commitment, we manage the Continuum of Care’s (CoC) Homeless Management Information System (HMIS), a vital data platform that helps member agencies track services, improve outcomes, and make data-driven decisions to better serve individuals and families in need.

In addition to managing HMIS, we provide ongoing support, training, and technical assistance to the wide network of agencies and partners administering direct homeless services. Through these efforts, we ensure consistent, high-quality care across our community’s homeless response system.

Building community awareness is also at the heart of our mission. We actively engage with local stakeholders, policymakers, and the public to educate and inspire collaborative action toward ending homelessness. Our belief is that a thriving, healthy Jacksonville is one where everyone has a safe, stable place to call home. We know that achieving this vision requires all of us working together—nonprofits, government, businesses, and citizens united in purpose.
