Transitions Homeless Center

Transitions Homeless Center, operated by the Midlands Housing Alliance at 2025 Main St in downtown Columbia, provides support for adults experiencing homelessness and a pathway toward stability and permanent housing.

Its work includes community outreach, a Day Center, meals, showers, laundry, computer access, case management, referrals, and housing planning. Community partners also connect people with healthcare, mental health and recovery support, legal aid, employment assistance, and other services.

Transitions offers programs designed to address varied needs, including support for veterans, young adults aging out of foster care, and older adults, along with emergency shelter, rapid rehousing, and permanent supportive housing initiatives.
